We are a Federally Qualified Health Center providing Primary Care, Pharmacy, Mental Health, and Dental Services.
All services are provided at discounted rates based on a sliding fee scale.

Increased the number of total patients by at least 5% and increased the number of patients receiving mental health, substance use disorder, vision, dental, or enabling services.
Screens for tobacco use and assists with cessation, uses statin therapy for prevention and treatment of cardiovascular disease, and controls high blood pressure for at least 80% of the population.
Adult Body Mass Index with a follow-up plan at least 86% of the population, and weight assessment and counseling for nutrition and physical activity at least 85% of the population.
